Undergraduate Awards Application Form

The application deadline (for both the online application form and recommendation letter) is 5pm, January 24, 2020.  Please request a recommendation letter at least one week prior to the deadline to give your reference adequate time to submit a letter.

  • Academic information
  • Name and contact information for one reference from a faculty member in a science discipline (you must request that recommendation letters be submitted directly by your reference to ibioscholarships@okstate.edu; the Selection Committee will not contact references for you)
  • Essay Questions (Please complete both)
  • Copy of your CDIB card (only if you are applying for the Edwin Chappabitty Jr. M.D.  Scholarship)

Recommendation Letter

A single letter of recommendation should be provided by a faculty member in a science discipline. For the Glass Award, your letter must be from a faculty member in the Department of Integrative Biology. Have letter e-­‐mailed to ibioscholarships@okstate.edu.

Essays

Each essay should be approximately 500 words.

By submitting this application, you are agreeing that if you are selected as an award recipient that you will attend the Integrative Biology Banquet on April 17, 2020 to receive your award.
