Undergraduate Awards Application Form

The application deadline (for both the online application form and recommendation letters) is 5pm, February 2, 2018.  Please request recommendation letters at least two weeks prior to the deadline to give your references adequate time to submit their letters.

Please complete each section below, including:

  • Contact information
  • Academic information, including an unofficial copy of your transcript with grades from fall 2017
  • Names and contact information for two references, including at least one faculty member in a science discipline (you must request that recommendation letters be submitted directly by your references to ibioscholarships@okstate.edu; the Selection Committee will not contact references for you)
  • Curriculum vitae (cv; described below)
  • Essay (described below; please note that applying for the Dr. Bryan P. Glass Scholarship/ Fellowship requires an additional essay)
  • Copy of your CDIB card (only if you are applying for the Edwin Chappabitty Jr. M.D.  Scholarship)

Please indicate whom you have asked to provide letters of recommendation. At least one of the letters must be from a faculty member in a science discipline. If you are applying for the Dr. Bryan P. Glass Scholarship/Fellowship, one letter must be from a faculty member in the Department of Integrative Biology. Letters should be sent via email to ibioscholarships@okstate.edu.

By submitting this application, you are agreeing that if you are selected as an award recipient that you will attend the Integrative Biology Banquet on April 18, 2018 to receive your award.
